From Route 86, make a left on to Springfield Road in Wilmington. Turn right on Community Circle Drive. The Health Center is located in the Town Office Building.

The goal of High Peaks Health Center is to offer established patients an urgent appointment within 24 hours.

High Peaks Health Center provides:

  • Health Care for all ages.
  • Diagnosis and management of routine health problems (high blood pressure, diabetes, heart disease, etc.)
  • Health maintenance and screenings including routine exams, pap smears, school/camp/work physicals.
  • Urgent care for injuries and illnesses.
  • A working relationship with a variety of physicians who provide specialty care.

After hours Emergency Care can be obtained through the Elizabethtown Community Hospital. Contact the Emergency Department at Elizabethtown Community Hospital at 873-6377.

Prescriptions
If you are followed routinely we can usually refill your medication without an extra office visit.

You can fill your prescription by calling the office at (518) 946-1111.

We require 48-72 hours notice for regular prescription requests. Controlled substances and certain pain and sleep medicine may take longer and must be picked up in person. Please give the exact name or spelling of the medication, the strength (mg, mcq, meq,) how and when taken and the number required

Billing & Insurance
  • Health Center billing is handled by Elizabethtown Community Hospital at 946-1111. If you have payment concerns the ECH Business Office may be able to help.
  • The Health Center participates with numerous plans and insurances.
  • If you belong to an HMO, (Health Maintenance Organization), you need to designate the Health Center physicians as your primary care provider.
  • Co-payments are due at the time of your visit.

How can I fill my prescription?
Call the Health Center at 946-1111.

What if I need an appointment right away?
The Health Center tries to hold a few openings every day for urgent needs. Call the evening before and leave a message or call early in the morning for availability.

What if I can not make a scheduled appointment?
The Health Center requires two hours prior notice to cancel.

My primary care provider is at High Peaks Health Center, but can I go to Elizabethtown or Westport Health Center since it is part of the same hospital?
No. Although both Health Centers are part of the same hospital system, they are two separate practices. Patients are asked to establish with only one health center in the interest of continuity of care and patient safety.

I had laboratory testing/radiology exams done. When can I get my results?
Although many laboratory and radiology test results are reported quickly, the results must still be forwarded to your provider for review. Staff will notify you of your results as soon as your provider has given them instructions to do so. If you do not hear from us in one week, please call the office.

What if I am not home to receive my results? Can you tell my family member?
If you wish us to leave a message with a family member or friend regarding test results, appointments, etc., you may sign a release to be kept in your chart designating the individual(s).

I have a question about billing. Who should I speak with?
Health Center billing is handled by Elizabethtown Community Hospital. For payment concerns or problems, the ECH Business Office may be able to help. Please call (518) 873-6377.

How can I obtain copies of my records?
Records may be requested via an approved HIPAA release form. All record requests will be filled within 10-14 working days after receipt of payment. There is a charge of $.75 per page for all records.

Will I be charged for copies of my records if I am transferring care to another physician?
No. We will send the last six months records free of charge to your new physician, but only for the first request. If you transfer to yet another physician, there will be a charge for all records.
