If you are experiencing financial challenges or are unable to pay your health care bill in full, University of Vermont Health Network offers a range of financial assistance options so you can continue to access the care you need, when and where you need it.

UVM Health Network is committed to providing financial assistance to patients who are uninsured, underinsured, ineligible for a government program or otherwise unable to pay for medically necessary care.

Apply for Financial Assistance

To apply for financial assistance, complete the application and mail it to the address on the form. Please include the requested supporting documentation found on the application. If you need help completing the application, contact UVM Health Network's financial assistance office at 802-847-8000 or toll-free at 800-639-2719.

Providers Offering Financial Assistance and Discounted Care

Not all providers are covered by our financial assistance policy. To find out if your provider is covered, see the participating providers list below. If your provider is not listed, they are not covered under our financial assistance policy.

Hospital Bills: All UVM Health Network eligible services are covered under the financial assistance policy.
